Our pledge regarding your PHI
We are required by law to maintain the privacy of your PHI, provide you with this Notice of our legal duties and privacy practices, and follow the terms of the Notice currently in effect.
How we may use and disclose your PHI
For treatment
Your clinicians use your PHI to provide, coordinate, and manage your care, including consultations with pharmacies and labs.
For payment
We use PHI to bill for services and obtain payment from you or your plan.
For health care operations
We may use PHI for quality assessment, training, credentialing, and internal operations.
To business associates
We share PHI with vendors that perform services for us, under written Business Associate Agreements (BAAs) that require them to protect your information. These include our EHR/scheduling provider, cloud hosting, and payment processor.
As required by law
We may use or disclose PHI when required by law, in response to public-health activities, lawful court orders, or to avert a serious threat to health or safety.
With your written authorization
Other uses, including marketing, sale of PHI, and most uses of psychotherapy notes, require your written authorization. You may revoke an authorization at any time, in writing.
Your rights
- Access: request a copy of your medical records.
- Amend: ask us to correct information you believe is incorrect.
- Accounting of disclosures: ask for a list of certain disclosures we have made.
- Request restrictions on certain uses or disclosures.
- Request confidential communications at a specific address or number.
- Receive a paper copy of this Notice on request.
- File a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate for filing a complaint.
Breach notification
If a breach of your unsecured PHI occurs, we will notify you as required by HIPAA and applicable state law.
Changes to this Notice
We may change this Notice and apply the new terms to PHI we already hold. The current Notice is always posted at neuvemd.com/hipaa.
