Telehealth Informed Consent
Last updated: June 20, 2026
What telehealth is
Telehealth means receiving care through secure video, phone, messaging, and digital intake forms instead of in person. Your clinician can evaluate your history, order labs, prescribe appropriate treatment, and coordinate follow-up — all remotely.
Benefits
- Care from home, on your schedule.
- Continuity with the same clinician across visits.
- Easier follow-up between traditional in-office visits.
Risks and limitations
- Without an in-person exam, some conditions cannot be fully evaluated and may require an in-person referral.
- Technology can fail; a dropped connection may delay care.
- Information shared electronically, although protected, can in rare cases be intercepted or disclosed in error.
You must be in Arizona
You must be physically located in Arizona at the time of any clinical visit. If you travel outside Arizona, care must pause until you return.
Not for emergencies
Telehealth is not appropriate for medical emergencies. If you are experiencing chest pain, suicidal thoughts, severe bleeding, or any other emergency, call 911 or go to the nearest emergency room.
Your records
Your clinical record is created and stored in your secure Healthie chart and is governed by our HIPAA Notice of Privacy Practices.
Your right to withdraw consent
You may decline or withdraw consent to telehealth at any time without affecting your right to future care. To withdraw consent, email hello@neuvehealth.com.
By booking a visit you confirm that you
- are at least 18 years old and physically in Arizona;
- have read and understood the benefits, risks, and limitations above;
- understand telehealth is not an emergency service;
- agree to receive care through telehealth from a NEUVÉ clinician.
